Requirements for Online Pharmacy Certification

OP Certification $4499

The OP will have to file all of the appropriate papers, and provide the following information:

• Names and addresses of pharmacies
• Names and addresses of Physicians and license #s by state
• The drugs the pharmacies will be filling for the OP
• The DEA#'s and the licensing of the pharmacies.
• Papers to ensure the pharmacy is adhereing to gov standards.
• Number of OP employees
• Name and address of OP
• OP Officers
• Proof of Process for Doctor patient relationship for controlled meds
• Provide proof that a process is in place to ensure meds are not ordered early
• Provide proof that a process is in place to ensure Fraud is identified
• Provide proof that the customer cannot re order after being declined by submitting different details eg BMI
• Provide proof that a process is in place to eliminate affiliate spam (Agrees to provide details of any changes to the above within 5 working days of the change)
• Review of up to 5 web sites

Any entity seeking to join the Internet Pharmacy Board ("IPB") must complete the following application form which will be reviewed by the board members for approval. Each entity must provide clear/acceptable answers to the questions on the application form. In addition the Privacy sStatement must be signed by all members of the IPB. All information disclosed on the application is used for certification/acceptance purposes only and will not be disclosed to third parties.
